Breast augmentation is the most common cosmetic surgical procedure performed in the United States, with over 300,000 women undergoing this procedure each year. However, while breast implants are extremely safe, they are not a lifelong device and must be removed and replaced through breast revision surgery at some point in your life. The History Of Breast Implants In The United States

Breast implants are one of the most studied medical devices in history. As such, they have at times been taken off the market by the Food and Drug Administration (FDA), revised, improved upon, and studied again in order to assure the safety of the products on the market.

After 14 years of research, silicone breast implants were reintroduced in 2006 with a new shell and improved texture. In addition to silicone, saline implants continue to be utilized in women for both aesthetic enhancements and for breast cancer reconstruction.

Deciding When To Revise Your Breast Implants

When considering the removal of your implants, it is important to discuss this with a board-certified plastic surgeon, like Dr. Parcells. Often, women will experience asymmetry, sagging, or drooping after removal.

Scar tissue often develops around the implant, called a capsule, which may also need to be removed during the procedure. Because of this, a revision procedure may need to be performed in order to give you the shape you desire. At Parcells Plastic Surgery in New Jersey breast revision surgery consists of the removal of your current implants and inserting fresh implants. When a patient does not wish to continue with implants a breast lift may be called for to achieve a shapely result.

Reasons For Breast Implant Revision

Other than the length of time, there are additional other reasons to initiate breast implant revision.

  • The scar that forms around the implant, called a capsule, may begin to harden or tighten, causing a condition called capsular contracture. This hardness can even migrate up to the armpits, causing discomfort and change to how the implants feel. This may happen years after implant placement and may happen to one or both breasts.
  • A trauma, such as a car accident or bad fall, can also cause the implant to rupture. In a saline implant, this will cause deflation as the saline is harmlessly absorbed by the body, leading to a smaller or deflated breast size. In contrast, a rupture to a silicone implant may not be immediately noticeable to you, as the newer silicone breast implants have a gel or gummy bear-like texture. These cohesive gels make it a bit more difficult to detect a rupture, as the breast appears the same in size.
  • Positional changes, which are caused by significant weight gain or loss, or by gravity and time, can cause changes to the appearance of the breast, leading to sagging. Often, a breast lift is required to maintain the aesthetic and youthful appearance of the breast.
  • Rippling, or wrinkling of the skin at around the breast edges is more common with saline implants than with silicone. This is usually followed by palpability, or being able to actually feel the implant through the skin. While this often occurs after long time use, it is generally a sign that the implant is ready to be replaced.

Breast Implant-Associated Anaplastic Large Cell Lymphoma - BIA-ALCL

Recently, there has been a lot of media attention regarding a condition called Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL).

As of October 2019, 573 cases of BIA-ALCL have been pathologically confirmed, and 288 have been suspected but are unconfirmed. It is important to note that BIA-ALCL is not a type of breast cancer, but rather, a type of lymphoma that has been linked mainly to textured silicone implants. In most cases, definitive treatment is the removal of the implant. As such, Dr. Parcells is a fully textured free practice.

Breast Revision Surgery And Recovery

On average, breast revision surgery can take 2-3 hours and is performed under general anesthesia.

The incision can be placed in a variety of locations, depending on the type of revision procedure being performed. The location of your previous scars is likely to be utilized for excision of your implants and implantation of new implants where possible. A numbing agent is often utilized to relieve discomfort, allowing most women to go home the same day. A surgical bra will be worn for the next several weeks to help with swelling and shaping. Most women experience mild to moderate discomfort for the next several days but may be able to resume routine activities or go back to work in about a week.

Breast Implant Revision FAQs

This is a great option for women with excessive (but deflated) natural breast tissue who wish to have their implants removed without replacement of another implant. As such, the excess breast tissue is reshaped to mimic the appearance of a firmer breast. Since this can sometimes result in a smaller cup size, it is important to discuss with Dr. Parcells if this option is right for you.

At this time, no. There are no long term efficacy studies looking at the safety of injecting fat into the breast gland.

Currently, neither the FDA nor any plastic surgery society recommends that women should preventatively remove textured breast implants to prevent BIA-ALCL.

However, there are women who are concerned enough and were already considering removing/replacing their implants, and this gave them one more reason to proceed. Women with textured implants may elect to have them replaced with a smooth implant, or may simply choose to have their breast implants removed entirely.

If a rupture is suspected, an MRI can help to diagnose this. Symptoms that may lead you to notice a rupture in a silicone implant include:

  • Change in breast shape
  • Swelling
  • Changes to sensation
  • Pain or tenderness of the breast

Breast Revision Costs

Removal of breast implants and capsule are sometime covered by insurance. For reconstruction patients, this is nearly always the case. For cosmetic patients, Dr. Parcells will assess your concerns and can give you her opinion on whether she believes you will qualify for insurance approval.
